Addressing the Challenge and Constraints of Insulin Sources and Supply – ACCISS Phase 3

Despite the discovery of insulin more than 100 years ago, nowadays, there is still a large gap to cover in its access, knowledge and acceptance by patients and health professionals. Since 2018, the ACCISS project in Peru has been working in topics related to the access to insulin in our country. To date, two phases of this project have already concluded [Phase 1 (Surveying insulin prices, availability and affordability in Peru and Evaluation of the response of the health system to the access to insulin for people with Diabetes Mellitus type 1) and Phase 2]

giving rise to this third phase where different components are being worked on in order to achieve the general objective of the project.

Aim:

Assess the facilitators and barriers that promote or hinder access to insulin in Peru.

 

Study design:

Based on the general objective, this third phase of the project is divided into the following components:

  • Monitoring of the availability and prices of insulin, administration devices and glucose self-monitoring devices in health facilities and homes in Peru. Data will be collected at different times in order to provide information to stakeholders on insulin access and use, as well as supply administration and self-monitoring. In addition, the information will contribute to a multi-country study on the availability, prices, and affordability of blood glucose self-monitoring devices.
  • Implementation of an insulin use manual in the public health system of a region of Peru. Due to the fact that educational programs have proven to be effective in improving results and reducing treatment costs, a hybrid implementation study with mixed methods has been proposed through a virtual training course for health personnel on the use of insulin and monitoring. of patients with diabetes. This course is based in the Tumbes region and will culminate with a face-to-face workshop on insulin application. The objective of this component is to evaluate the implementation process of the training course and the education manual for the application of insulin in public health establishments of the different levels of health care in the Tumbes region.
  • Co-creation for the identification of a set of outcomes of interest for type 1 diabetes. This component seeks to use co-creation to create a set of basic results including the perspectives of patients, caregivers, health personnel and stakeholders. stakeholders considering type 1 diabetes mellitus as a model.   • Towards the participation of various actors in health decisions: the case of diabetes mellitus in Peru. In order to describe the barriers and facilitators for citizen participation in decision-making processes in health policies in Peru, a qualitative study will be carried out with in-depth interviews with various key actors including people with type 2 diabetes mellitus. 1 and their caregivers, representatives of professional societies, entities involved in work with civil society, researchers and government officials, among others.
